Custom Half Sleeve

Summary:

I'm looking for a half sleeve design that includes 4 key elements and the rest is pretty open for you to be creative.

Description:

I'm looking to get a custom half sleeve for my right arm done with the following elements: 

- A tree will be the central element and should extend vertically up my bicep with the round part (leaves) kind of filling the top of my arm.  So the trunk will be in the bicep area and the upper branches/leaves will fill the top part of my arm where it connects to my shoulder. 

- On either side of the trunk, I'd like my parents names in Western (like old saloon) type font.  

- An american flag as part of the background. 

- On that part of the sleeve that reaches the top of my chest I'd like to have the date 09/12/2011 put in.  I'd like to add more dates below that so please keep that in mind.  

Since its a half sleeve there is a lot of space to fill and this is where I'm open.  Feel free to be creative.
